At the time of initial registration, families must present the following:

• $75.00 Application Fee
• Baptismal Certificate (original or copy must be supplied only if child was not baptized at St Agatha Parish)
• Birth Certificate (original or copy)
• Custodial Documentation (a certified copy of any child custody order or decree pertaining to the child must be presented at the time of admission)
